Services Agreement

Amanda M. Cobert, doing business as Connect Your Dots

This Services Agreement (the “Agreement”) is between Amanda M. Cobert, doing business as Connect Your Dots (“I,” “me,” or “my”) and the person signing below (“you” or “Client”). It applies to any consulting session or service I provide to you (the “Services”). Please read it carefully. If anything is unclear, ask me before you sign.

1. Who I Am

I am a parent of a child with special needs, and I started Connect Your Dots to share what I have learned along the way. My perspective comes from lived experience. I am not a licensed medical, legal, mental health, social work, or education professional.

2. What I Provide

The Services are one-on-one conversations by phone, video call, or in person, in which I:

  • share my perspective and what has worked for my own family;
  • talk through options that may be available to you; and
  • suggest possible next steps and questions you may want to explore.

3. What I Do Not Provide

The Services are for general information and support only. I do not provide, and nothing I say should be taken as:

  • medical advice, diagnosis, or treatment;
  • therapy, counseling, or mental health services;
  • legal advice, or advice about your child’s legal rights;
  • financial, insurance, or government-benefits advice; or
  • special education advocacy or representation.

I do not attend IEP, school, medical, or agency meetings. I do not contact schools, providers, or agencies on your behalf, and I do not prepare or file paperwork for you. Working with me does not create a doctor-patient, therapist-client, attorney-client, or similar professional relationship, and conversations with me are not legally privileged.

About evaluations and results. I may share what a similar diagnosis, score, or result meant in my own experience, the same way I share other parts of my story. I do not explain, interpret, or tell you what your child’s evaluation, diagnosis, or test results mean, or what they imply for eligibility, treatment, or next steps — for that, please talk to the professional who provided them.

Referrals and mentions. If I mention a specific provider, school, product, or service, that is not an endorsement or a guarantee, and I am not responsible for the quality of their services. Please do your own research before deciding to work with anyone I mention.

4. Your Decisions Are Yours

Every family is different. What worked for mine may not work for yours. You are responsible for your own decisions and your child’s care. Please check anything important with the qualified professionals in your life (your child’s doctors, therapists, school team, or an attorney or advocate) before you act on it, and please do not delay or ignore professional advice because of something I said. I do not promise or guarantee any particular result.

5. Emergencies

I am not an emergency or crisis service. If you or someone in your family is in immediate danger, call 911. If you are in emotional crisis, call or text 988 (the Suicide & Crisis Lifeline in the U.S.).

6. Fees and Payment

Fees are listed on my website and are due before the session begins. Payment is accepted by check, Venmo, or Zelle. The price is the full price; I do not charge hidden fees. Any sales tax or processing fees that apply will be shown before you pay.

I may increase my fees with at least 60 days’ written notice. A fee increase will not apply to sessions or packages you have already paid for.

7. Refunds

Fees are non-refundable once paid, including for sessions or packages you do not use. The one exception is if this Agreement ends (see Section 12) — in that case, any prepaid fees for sessions that have not yet happened will be refunded.

8. Privacy and Confidentiality

What I keep private. I will keep what you share with me confidential. I will not share it with anyone else without your permission, except (a) where the law requires it, or (b) if I reasonably believe a child or another person is at serious risk of harm and I decide, in good faith, to contact the appropriate authorities.

If you share documents with me. If you choose to send me anything in writing (such as notes, records, or summaries), I will use it only to prepare for or support our conversation, keep it secure, and delete it within 30 days of our last session unless you ask me to delete it sooner. You can ask me to delete anything you’ve sent me at any time. I will not forward it to anyone else without your permission.

If your information is ever exposed. I take reasonable steps to keep what you share with me secure, but no system is completely secure, and I cannot guarantee against unauthorized access. If I ever learn that something you’ve shared with me was accessed or disclosed without permission, I will tell you as soon as I reasonably can.

Not a healthcare provider. I am not a healthcare provider, and the Services are not covered by HIPAA.

Contact and payment details. I will collect only what I need to schedule sessions and take payment (such as your name, email, and payment information). Payment information is handled by my payment processor, and I do not store card numbers.

No recording. Neither of us will record a session without the other’s written permission.

9. Your Story and My Marketing

I will not use your name, your child’s name, your story, or anything you say in a session in my marketing, social media, or public materials without your separate written permission.

10. My Materials

Any handouts, guides, or other materials I give you are for your personal, non-commercial use. Please do not resell, post publicly, or distribute them without my written permission.

11. Limits on My Responsibility

To the fullest extent the law allows, I am not responsible for any indirect, incidental, special, or consequential losses arising from the Services or from decisions you make based on them. To the fullest extent the law allows, my total responsibility to you for any claim related to the Services is limited to the fees you paid me for the Services in the three (3) months before the claim arose. Nothing in this Agreement limits responsibility for fraud, intentional misconduct, or anything else that cannot legally be limited.

12. Ending This Agreement

Either of us may end this Agreement at any time by written notice (email is fine). If we end it, you will owe nothing further for sessions that have not yet happened, and I will refund any prepaid fees for those sessions. Sections 3, 4, 8, 9, 10, and 11 continue after the Agreement ends.

13. The Fine Print

  • Whole agreement. This Agreement (with any booking confirmation) is our entire agreement about the Services and replaces anything said before.
  • Changes. I may update this Agreement for future Services by giving you notice. Changes will not apply to sessions you have already booked unless you agree. Any change to an existing booking must be in writing.
  • If part is unenforceable. If a court finds any part of this Agreement unenforceable, the rest stays in effect.
  • No transfer. You may not transfer this Agreement to someone else. I may transfer it only to a successor to my business, and I will tell you if I do.
  • Electronic signatures. An electronic signature or a click of an “I agree” box is as valid as a handwritten one.
  • Communications. We may use email and other electronic means to communicate. Email and messaging are not a secure channel, so please keep that in mind when deciding what to write.

14. Agreement

By signing below or clicking “I agree,” you confirm that you have read and understood this Agreement, that you are at least 18 years old, and that you agree to it.
